Measuring blood pressure at home so the numbers mean something

Home blood pressure monitoring predicts cardiovascular events at least as well as clinic measurement, and it removes white-coat effect. It also produces some of the least reliable numbers in medicine when done casually.
The technique is not complicated. It is just specific, and small deviations move the reading by clinically meaningful amounts.
The errors that cost you the most mmHg
Each of these has been quantified in validation studies, and they add up quickly.
- A cuff that is too small: adds 2–10 mmHg. Measure your upper-arm circumference and match the cuff
- Cuff over clothing: adds 5–50 mmHg depending on thickness
- Unsupported back: adds 6–10 mmHg
- Legs crossed: adds 2–8 mmHg
- Arm below heart level: adds up to 10 mmHg — resting it in your lap is a common mistake
- Talking during the reading: adds 10–15 mmHg
- A full bladder: adds up to 10 mmHg
The protocol
Follow the same sequence every time so readings are comparable across weeks.
- No caffeine, smoking or exercise for 30 minutes beforehand; empty your bladder
- Sit with back supported and feet flat on the floor for five quiet minutes
- Bare upper arm, cuff at heart height resting on a table
- Take two readings a minute apart and record both; average them
- Measure twice daily — morning before medication, evening before bed — for seven days
- Discard day one, then average the remaining readings
Home thresholds are lower than clinic thresholds
Because home readings lack the white-coat effect, the diagnostic cut-off is lower. A clinic threshold of 130/80 corresponds to roughly 130/80 for daytime home averages under ACC/AHA, while European guidance uses 135/85 for home versus 140/90 in clinic. Check which convention your clinician follows before comparing.
Choosing a device
Use a validated upper-arm oscillometric monitor. Wrist and finger devices are substantially less accurate in routine use. Independent validation listings such as STRIDE BP and Validate BP publish which specific models have passed testing — the brand alone is not enough, since manufacturers sell validated and unvalidated models side by side.
